- Unresponsive to cardioversion
- Manage Underlying Causes: lung disease, theophylline, hypoxemia, lactic acidosis, CHF, renal failure
- Calcium Channel Blockers: useful
- Amiodarone (Type III agent with modest beta-blocking properties): useful for cases with rapid HR-
